Concierge Care Network - CCN provides professional care coordination, guidance and support for older adults, their families and caregivers that focuses on maintaining wellness and enriching the quality of life.  We are an interdisciplinary team of professionals who provide supportive counsel and oversight on needs, diagnoses management, resource assistance and the many other key issues that present with aging. 

 

We are committed to ensuring that our clients receive the needed support and care in the place they call home. 

This is accomplished by:

  • Conducting a comprehensive personalized assessment which will focus on your specific goals and preferences. 

  • Developing and implementing a customized wellness plan tailored to your needs and priorities

  • Connecting you to community resources

  • Coordinating and managing all aspects of your care and well-being

  • Providing ongoing monitoring of the progress through home visits, virtual calls and telephone calls to adjust the plan as needed.
